Instructions for Youth Opportunity Partner Organizations:  Please submit the Affiliate Agreement Registration Form below and submit your organization's list of participants (including any applicable coaches).  Please note we will not provide enrollment instructions for your organization's designated participants until receipt of full payment (or written confirmation of a fee waiver).
